Submit the Ammunition Statement and a copy of your photo ID with your order.
Ammunition statements will be kept on file for future orders.
Submit the Automatic Knife Statement and a copy of your law enforcement, military, EMT, Fire, or Rescue personnel identification with your order.
Bona fide dealers, please send the completed form with a copy of your business license, state sales tax registration certificate, and a legible copy of your photo ID.
For California residents only, submit the Impact Weapon Statement with a copy of your law-enforcement or security guard identification with your order.
State and local law-enforcement agencies must submit the FET Exemption Form with your order. If completed properly, FET forms will be kept on file and can be used for future orders placed within 12 calendar quarters.
